First Visit

Below are the activities you can expect on your first visit chiropractic visit to Montgomery Chiropractic & Wellness Springs (MCWS).

COMPLETE PAPERWORK

On your first visit we will ask you to fill out some brief paperwork concerning your medical history, which gives us important information about yourself and your past medical conditions. This will help us to better prepare for the examination and any required treatment that you may need.

To get a jumpstart on your chiropractic care, you can download the appropriate forms by clicking on them below, fill them out, and bring them with you on your first visit to MCWS. To open the forms you will need Adobe Acrobat Reader.

• New Patient Form - This form is for all "new" chiropractic patients to MCWS and it is necessary to fill this form out for all patients
  
  
• Job Related Injury Form - This form is for any patient who requires chiropractic care as a result of an injury on the job

Introduction

Once your paperwork has been completed for chiropractic care, you will have a one-on-one meeting with Dr. Montgomery to discuss your health-related problems and concerns, and answer any questions you might have.

The introduction is primarily to better understand your current health conditions and to educate you on the myths and facts about chiropractic treatment.

History & Examination

In order to diagnose your circumstance requiring chiropractic care, Dr. Montgomery will ask you a series of questions related to chiropractic conditions and will conduct specialized exams to determine the severity of your condition. A chiropractic exam will also consist of orthopedic and neurological tests and an evaluation of your posture.

Same Day Treatment

If necessary, chiropractic patients may receive treatment following the examination. The treatment normally consists of an alignment of the backbone, or other joints in the body, and incorporates heat or cyro (ice) therapy, Trigger Point Therapy and soft tissue work. Dr. Montgomery often incorporates soft tissue work with the adjustment. This combination helps to reduce the built-up stress and tension of the body leading to better patient relief.

Post Exam Consultation and Home Care Program

After the exam and chiropractic treatment (if necessary) Dr. Montgomery will provide a full "post-exam" consultation for you. Dr. Montgomery will explain the prescribed treatment for recovery of your condition. This may also include a personalized Home Care Program that will cover things such as how to sleep, how to sit properly at work, how to lift items, etc. The program may also include basic exercises, stretches, ice or heat application and instructions regarding certain activities or positions to avoid. If follow-up visits are needed they will be scheduled at a time that is convenient for you.
